Clinically, "back pain" can involve a combination of different mechanisms, which a practitioner will distinguish through inquiry and examination. The following is for general understanding only – the same sensation can have different causes; SYT Wellness will first diagnose before deciding whether acupuncture, Tui Na, bone setting, or traumatology takes priority.

Commonly caused by prolonged poor posture, weak core muscles, or repetitive bending. Often presents as lower back soreness, dull pain, worsening after activity. Most common in office workers and drivers.

Acute Sprain · Sudden Back Lock

Severe pain after a sudden turn, lifting heavy objects, or sneezing, with significantly limited movement. Traumatology external treatment and appropriate rest are particularly important.

When should you seek emergency care or Western medical attention first?


TCM pain clinics are suitable for most urban and strain-related back pain. However, if you experience any of the following "red flag symptoms," please prioritize seeking emergency or Western medical evaluation and do not delay self-treatment:

  • 🚨Severe abdominal pain, chest pain, difficulty breathing, or pain sensation significantly different from general "lumbar muscle" pain
  • 🚨Sudden severe back pain during pregnancy accompanied by other obstetric warning signs

How does SYT Wellness treat back pain?


The core principle is to diagnose first, then treat: understand the cause of pain, course of illness, degree of activity limitation, and body constitution, then combine therapies, avoiding a singular approach of "treating pain with only pushing or needling."

TCM back pain assessment usually includes:

Inquiry (triggers, nature of pain - soreness, distension, sharp, radiating, nocturnal pain; past medical history and lifestyle), inspection, auscultation, palpation, assessment of lumbar spine mobility and tender points, preliminary examination of related muscle tension and lower limb sensation/strength, and recommendations for the day's plan and follow-up frequency.

The treatment combination is determined by the practitioner after assessment on the day. Common approaches include:

Acupuncture for Back Pain

Applying needles to the lower back, buttocks, and related meridian acupoints to clear qi and blood, aid in reducing inflammation and pain, and relieve local tension. It has an auxiliary regulating effect on acute lower back sprains, chronic strains, and sciatica radiating discomfort, and is often used in conjunction with Tui Na.

Common sensations: Slight distension or radiating sensation along the meridians, acceptable to most people. If uncomfortable, inform the practitioner immediately for adjustment.
2 Muscle relaxation & spasm relief

Tui Na for Back Pain

Targeting the lumbar fascia, erector spinae, and gluteal muscles, Tui Na manipulation techniques loosen tense tissues and improve lumbar spine mobility, alleviating pulling discomfort. Often used in conjunction with acupuncture to enhance efficacy.

Suitable for: Stiff lower back, limited mobility, muscle tightness after prolonged sitting, and acute exacerbation of chronic back pain.

Bone Setting for Lumbar Spine

In suitable cases, manual bone-setting techniques are used to adjust misalignments or limited mobility of the lumbar spine and pelvis, helping to restore correct lumbar biomechanics and reduce recurrent episodes.

Important: Bone setting is not suitable for everyone. The practitioner will first assess for contraindications (e.g., osteoporosis, acute nerve compression); for those unsuitable for strong manipulation, milder alternative approaches will be used.

Traumatology External Treatment

For acute sprains, bruises, or chronic bone-related back pain, external application of traumatology plasters or herbal compresses directly acts on the affected area, aiding in invigorating blood circulation, reducing swelling, and relieving pain. Depending on the condition, oral Chinese medicine may also be combined.

Acute phase of back sprain: Timely traumatology intervention often significantly shortens recovery time and reduces swelling and pain.
The actual treatment combination will be determined by the practitioner after assessment on the day. The goal is to alleviate pain, improve mobility, and help you return to a manageable daily rhythm. We do not make promises of "guaranteed cure" or fixed treatment durations.

When can improvement be expected with TCM treatment for back pain?


Improvement time varies depending on the type and severity of the condition, as well as individual constitution. The following is a general reference and not an individual guarantee:

Acute Back Sprain · Muscle Strain

After timely assessment and traumatology/acupuncture treatment, some individuals may experience significant pain relief and restored mobility in a short period. However, it is still necessary to avoid immediate strenuous activity to allow the body adequate recovery time.

Often requires phased follow-up, along with adjustments to posture, activity, and sleep. Improvement is often a "step-by-step" progression rather than immediate, requiring active cooperation from the patient.

Daily Back Care Advice (General Conservative Principles)


The following are general conservative principles applicable to most urban-type back pain. If you have any questions, please confirm with your practitioner during your consultation whether they are suitable for your condition:

Ensure adequate lower back support and keep both feet on the ground when sitting. Stand up and stretch every 30-45 minutes to avoid prolonged slouching or crossing legs.

During acute severe pain, reduce sudden twisting or heavy lifting; after relief, gradually resume low-impact activities like walking. Avoid overcompensating with intense exercise all at once.

For acute sprains, brief ice application (10-15 minutes each time, with a towel) may be used for the first 48 hours; for chronic stiffness without obvious swelling or heat, warm compresses are more comfortable. If unsure, consult your practitioner.

Choose a mattress that supports the natural curve of your lower back; if sleeping on your side, place a thin pillow between your knees to help reduce lower back twisting and morning stiffness.

Avoid vigorous self-"cracking," blindly following difficult stretches, or performing aggravating movements like sit-ups when in severe pain.

Frequently Asked Questions about TCM Treatment for Back Pain


Is it always necessary to have X-rays or surgery for back pain?

Not necessarily. Many urban-type back pain cases are related to posture, fascial strain, or restricted mobility, and do not require surgery or imaging. SYT Wellness will first conduct a TCM assessment and then recommend approaches such as acupuncture, Tui Na, bone setting, or traumatology. If symptoms suggest the need for further imaging or surgical evaluation, we will truthfully recommend consulting a Western doctor to ensure you receive appropriate treatment without delay.

Does acupuncture for back pain hurt much? What does it feel like?

Needle sensation varies from person to person, but it is generally well-tolerated, often described as a slight distension, a radiating sensation along the meridians, or localized warmth. The practitioner will select acupoints based on your constitution and pain condition, and communicate with you throughout the process to ensure your comfort. If you feel any discomfort, you can inform the practitioner immediately for adjustments.

How long should back pain last before seeking TCM treatment?

If back pain persists for several days without easing, affects sleep or daily work, radiates to the buttocks or lower limbs, or recurs frequently, it is advisable to seek assessment early and not delay. If red flag symptoms appear (such as loss of bladder/bowel control, progressive lower limb weakness, severe pain after trauma), you should immediately seek emergency or Western medical attention.

Is bone setting effective for back pain? Is it suitable for everyone?

Bone setting can be an adjunct for certain types of lumbar spine mobility restrictions or minor misalignments, but it is not suitable for everyone. The practitioner will first assess mobility and any contraindications (e.g., osteoporosis, acute severe disc compression); for those unsuitable for strong manipulation, milder auxiliary methods will be used as alternatives, often combined with acupuncture or Tui Na.

Can TCM help with sciatica?

Discomfort radiating from the lower back to the buttocks, back of the thigh, or calf (commonly known as sciatica) can be caused by various factors. Acupuncture, Tui Na, and bone setting, as auxiliary treatments, can be helpful for some individuals, but a complete assessment of nerve symptoms and mobility is required first. If symptoms are severe (e.g., significant muscle weakness, continuous worsening), it may be necessary to combine with Western medical imaging to confirm the cause before proceeding with treatment.
